01Acceptance of terms
These Terms and Conditions govern your use of the websites, landing pages, forms, and communication programs operated by Optima Ophthalmic Medical Associates, Inc. ("Optima," "we," "us," or "our"), located at 1237 B Street, Hayward, CA 94541. By visiting our site, submitting a form, or opting in to receive messages from us, you agree to these terms and to our Privacy Policy.
If you do not agree with these terms, please do not use our website or submit your information.
02Eligibility
You must be at least 18 years old to submit a form, request a consultation, or consent to receive text messages from us. If you are inquiring on behalf of a minor, you confirm that you are the parent or legal guardian and that you are authorized to provide the information and consent you submit.
03No medical advice and emergencies
Content on our website and in our messages is general education only. It is not medical advice, and it does not create a physician and patient relationship. Candidacy for LASIK, PRK, EVO ICL, custom lens replacement, cataract surgery, or keratoconus treatment can only be determined through an in person examination and diagnostic testing.
If you are experiencing sudden vision loss, flashes and floaters, eye trauma, severe eye pain, or any other medical emergency, call 911 or go to the nearest emergency department. Do not use text message, email, or a web form to report an emergency.
Individual results vary. Statements about outcomes, recovery, or reduced dependence on glasses and contact lenses reflect typical experiences and are not a guarantee of your result.

05Email and phone communications
When you submit a form or contact us, you agree that we may respond by phone, email, or text message using the contact details you provided. Marketing emails include an unsubscribe link in every message. Unsubscribing from marketing email does not stop transactional messages such as appointment confirmations, clinical instructions, or billing notices.
Calls to and from our office may be recorded or monitored for quality and training purposes where permitted by law. You will be notified at the start of the call when recording applies.
06Appointment requests and consultations
Submitting a form is a request, not a confirmed appointment. An appointment is confirmed only when our team confirms it directly with you. Please give us reasonable notice if you need to cancel or reschedule, so we can offer the time to another patient. Fees, quoted pricing, and promotional offers are subject to change, subject to candidacy, and may be limited by time or by procedure type.
